Things to bring to your first appointment include:
Workers Compensation Injury or Auto Accident Related Conditions. Please have all the specific information available such as:
Length of Visits. As every patient's condition is different, your evaluation and treatment time allotted is unique to you! In general you can expect to be in the office for your initial appointment between 60–75 minutes. Subsequent treatment expectations and times will be discussed thereafter. Please inform our staff If you have time constraints or special circumstances and we will be happy to discuss options and accommodations.
Payments. Something about being due at time of service, copays, etc.? We understand the rising costs of healthcare and offer convenient payment options and payment plans.
Rescheduling Visits. Please contact our office is soon as possible if you need to alter your appointment.
Cell Phone Use. We ask that you please refrain from utilizing your cell phone in the clinic for patient privacy and HIPAA purposes.

- A list of your medications, x-rays, CT scans, MRI scans or any other studies that relate to your condition.
- A referral or any other pertinent medical records (if applicable)
- Photo identification
- Insurance card
- Please bring or wear something comfortable for your visit
